Care coordination is the process of helping you get the care you need, when you need it. There are many different types of care coordination services. WAHA offers three different kinds:

1. Care Transitions. Helping people make a safe transition from PeaceHealth St. Joseph Hospital to the home or nursing facility after being admitted. Click here to learn about Whatcom IMPACT Project.
