What is Primary Care?
A Primary Care Provider (PCP) is a health care provider who is a person’s primary point of care, working with the person to create a “medical home.” A primary care medical home is a patient-centered model of health care, in which the patient, his/her primary care providers, and other health professionals work together and with shared responsibility for the patient’s health.
