Access Coordinators

Access Coordinators offer the following free “CARE” services to help patients and their families navigate the health care system:

• Coverage: Assistance with enrollment in Medicaid, Children’s Health Insurance Plan (CHIP), Basic Health Plan and other insurance programs
  
  
• Access: Assistance establishing a “medical home”, i.e. ongoing access to a primary care physician or provider
  
  
• Resource: Counseling about available support services, including transportation, translation, and pharmacy assistance
  
  
• Education:  Ongoing assistance and support to patients and their families so that they can become more informed and engaged in getting health care.
